1. What is your current height and weight? 5 ft 6
2. If you were at an ideal weight now, what would that weight be? 57-58kgs
3. At what weight would you like to be at four months from now? 57-58kgs
4. Why do you want to lose weight? put on a heap over Xmas and want to be more active with my son and be competitive at sports again.
5. Do you want to lose weight for a specific life event such as wedding or reunion? If so, when is that event? Also wish to lose weight for family reunion in Australia in Nov 2010
6. What obstacles could get between you and your weight loss goals? myself and willpower...invited to lots of functions and this can be very tempting!
7. Why do you think that you now have a weight problem? Don't have a problem, just a bit overweight due to overindulging over the last 6 months.
8. What lifestyle changes do you think would help you lose weight? Possibly making healthier choices when socialising and stopping at 3 glasses of wine?
9. Have you lost weight in the past? If so, what has worked in the past to help you lose weight? Training for competitions/tournaments has helped to keep the weight off.
10. Why do you believe that you did not lose weight or you gained the weight back? Never really been on a proper diet and not really had problem losing weight, but a bit older now and have to work at it a bit harder.
11. What, if anything, has not worked for you in the past in helping you to lose weight? Why do you think it did not work? N/A
12. Would you try writing down all food and drink consumed for a given period of time? Yes as the consumption shock factor alone will encourage me!
13. Do you cook at home often? If so, what do you cook? Yes, normally something quick and easy...sometimes nutritious, sometimes not...depending on energy levels.
14. How often do you go out to eat? Where do you go? Eat out at restaurants and often try something new each time. Take outs is usually the Kebab shop.
15. What are your three favorite foods? Crayfish, prawns, oysters...most seafood!
16. What are your three favorite restaurants? Anything serving fresh Seafood, indian or Thai food restaurants.

7. What are three things you can do differently when it comes to food? Choose a healthy option or drink a glass of water before deciding on what to have.
18. If you woke up tomorrow and your body was exactly the way you want it, what would be different? Stomach would be back the way it was a year ago - flat! Thighs and bum would be back to toned and slimmer...wouldn't change anything else....perhaps bust a bit bigger, but only surgery can fix that!
19. Do you eat when you are not hungry? Sometimes.
20. Do you binge eat (large amounts at a time)? Sometimes, but it is usually chips!
21. Do you hide your food or eat in secret? No, can't say I do that!
22. Do you eat when you are sad, nervous, or depressed? Not really aware of it, but probably more so if I feel down.
23. Do you eat as a reward? Treat myself to a crayfish each year for my birthday, does that count?
24. Do you eat while watching TV or using the computer? Yes, while watching television...not while on computer...don't want crumbs in the keyboard!
25. What do you normally eat for a meal? Steak, chicken or fish and salad or veges, usually meat and three veg.
26. What type of snacks do you eat? Range from chippies, fruit or bikkies.
27. In terms of exercise, what, if anything, are you currently doing? Playing touch rugby and netball, and also coaching sports and walk regularly. Also have a vibration trainer at home which I haven't used for a while.
28. Where do you go for exercise? A local public gym? School/work gym? Home? Home, local schools and parks.
29. What, if anything, are your three favorite types of exercise?
Playing sports (touch rugby, netball and volleyball) and walking.
30. What is your daily/weekly/monthly/yearly motivation to move towards your goals? My reunion invite, wedding photo and photos of my rep sports teams.
31. Do you have rewards for certain goals? Will buy some new clothes each time I lose 5 kgs.
